Summary: retired chief air accident inspector in charge of the New Zealand Office of Air Accidents Investigations from 1975 to 1990 who was involved in several major aircraft accident investigation, including a controversial six-month inquiry as the chief investigator of the Air New Zealand DC10 crash at Mount Erebus, Antarctica in November 1979, in which all 257 people on board were killed.
